Adding 5 Questions to Survey... 1. All Needs Assessment surveys have been ACTIVATED by the State 2. If you have already LAUNCHED your survey to the public (# responses are posted), you can no longer edit nor add questions to your survey If you have NOT launched your survey, you may add questions to survey by clicking on “Activate” tab at top of screen 4. Click on “deactivate” survey

Double-check your work by clicking on “See Response Format” Example of what you will see
After you have added up to 5 questions, then click on “Activate” tab at top of screen
Check the box “or elect that respondents will not have pin assigned (OPEN) box Click on “Activate this Survey”

Launching Needs Assessment Survey to Students, Parents, Teachers Direct those taking the survey to this URL: http://www.counselorsroom.org/UtahDD. “DD” is replaced by your district number. (Example: for Alpine School District schools the URL is: http://www.counselorsroom.org/Utah1. Your link can be added to your webpage for participants to take the survey on their own computer. ***If you do not know your district number, please contact your lead counselor or CTE Director. We suggest that you provide contact information to your survey takers Go to your My Account tab Edit your email and / or your phone number Edit your options (just below the phone number)

Filter Features… Why would we choose exclude a featured group? Look below for an example: I chose to exclude students whose parents’ highest level of education was 2+ years of college. Why?
I wanted to know if there was a difference of aspirations between Students whose Parents completed 2+ years of college Vs. Parents who did not complete 2+ years of college…… I found results to be… Thus, my conclusions are…

The results of these interventions are shared with school community including school board
