Creating a Revolution to Drive Healthcare Transformation Russ Branzell, FCHIME, CHCIO CEO/President November 2014

Financial Level: Defcon 1

Care Improvement Process Status Quo – Fee for Service Wellness Care Quality Care Managed Care Value Care

HITECH Legislation Four Main goals of HITECH 1.Promote the use of IT in the US healthcare system 2.Establish national health IT standards 3.Incentivize the use of such standards in the delivery of healthcare 4.Improve the privacy and security of patient health data while modernizing the US healthcare system

The Meaningful Use Plan MU was originally meant to be a six-year program, occurring over 3 “Stages” or phases Data capture and sharing Advanced clinical processes Improved outcomes Stage 1 Stage 2 Stage 3

Construction Analogy Stage 1

What has MU Accomplished?

Data from CMS September 2014, October 2014

Construction Analogy Stage 2

Next Steps 1.Continued Progress of Meaningful Use  2014 program year has been very difficult Nationwide software update required by regulations More difficult Stage 2 objectives This has led to dangerously low participation in 2014, and 2015 is not shaping up much better 2.Interoperability  Intent of incentives were to stimulate adoption, but real challenge is interoperability and “meaningful exchange” of data 3.Standardization  Multiple versions of standards exist and implementations are different across care settings 4.Workforce and workflows  Dissatisfaction with EHR usability is growing 5.Care Coordination  “Culture of coordination” has not yet emerged, even where technology tools have matured

Building Blocks of HIT 2.0 Healthcare Information Exchange Certified Electronic Health Record Patient Portals Data Warehouse Business Intelligence Complete Data Integration Data Analytics World-class IT Team Unified Communications ACO System Population Health System Document Management

Digital Divide is Widening

Construction Analogy Stage 3

1) Integration 2) Value (Cost & Quality) 3) Transparency U.S. Healthcare - Three Process Trends

versus Evolution: gradual development of something, especially from a single to a more complex form. Revolution: fundamental change in a way of thinking about or visualizing something. EVOLUTION REVOLUTION vs.

Enabling Behavior Change:  Providers  Patients

Care Improvement Process Status Quo – Fee for Service Wellness Care Quality Care Managed Care Value Care

1.Don’t accept the status quo 2. Semper Gumby: “Always Flexible” 3. Plagiarism is skill, not a crime 4. Don’t evolve, REVOLVE!!!! Rules of the Revolution

Thank You!
