3 Bracknell and Ascot Clinical Commissioning Group Our conversations so far has led to priorities…  Patients feel supported and empowered to manage their own conditions  Professionals have access to health records to give more holistic integrated care  Appropriate appointments for patients with complex conditions to support self management of their care  Practices work closer together in Bracknell and Ascot  Work closely through co-commissioning with NHS England to improve premises, workforce, prevention of ill health and improving quality  Practice delivering together minor surgery and arrhythmia services
