For urgent medical problems, an appointment with a practitioner will be given. Please telephone on (0161) 432 0671 or call in to make an appointment. NHS Direct: 0845 4647 – For health advice and information, NHS Direct offers services 24-hour support and advice for any health worries or queries. WHEN WE’RE CLOSED / OUT OF HOURS To obtain help outside the normal surgery opening hours, please call Stockport Doctors’ Co-operative on 0161 476 2299, who are able to deal with your emergency needs between 6:30pm and 8:00am weekdays and throughout the weekend. The following services ARE NOT provided by the Co-operative: Any medication for the treatment of drug addiction Repeat Prescriptions 2nd Opinions Sick notes and medicals Contraceptive services other than the morning after pill (at the doctor’s discretion) ACCIDENT & EMERGENCY A&E is a 24-hour accident & emergency only service.

Appointments We will endeavour to offer you an appointment with your doctor of choice, however, in the case of urgent appointments you will be offered an appointment with the first available Doctor in your practice. If you are unable to keep an appointment, please let us know in good time, to allow us the opportunity of offering the appointment to another patient. We also offer pre-booked telephone appointments with Doctors. You can book appointments online – just ask at reception or go to www.hmmc.info for more details. Appointment slots are allocated on a per person basis. What do I do if I need an Urgent Appointment? If you need an urgent appointment, please contact the Surgery on (0161) 432 0671 as early as possible, you will be put on a computerised list and a nurse will contact you within the hour (Nurse Triage). Please ensure you give the receptionist a contact number. Nurse Triage This service is provided by both practices to ensure that all patients receive appropriate health care as soon as possible. This may be an appointment with a GP, a nurse practitioner, a practice nurse, the treatment room nurse or a home visit. In some circumstances the triage nurse may be able to give you advice on how to care for the problem yourself. If you phone the practice to ask for an appointment with a doctor or a home visit on the same day, you will be asked by the receptionist for a contact phone number so that one of the nurses can phone you back. The triage nurse will ask you for more information about the problem so that she can ensure that you see the most appropriate member of the healthcare team. This information will also help her to assess the urgency of the problem which could be vital if you or one of your family are seriously ill. All the nurses are experienced and have received special training to provide this service. The nurses are also happy to advise you if you are not sure whether you need to see the doctor or just need health care advice. Our GPs and large support teams offer a varied range of services to our patients. The surgery opened in 1971 taking in just six doctors originally. The practice was totally renovated and extended in 1994 and extended again in 2002 and 2009 We are an innovative practice and are always looking for ways to improve our services for patients, through participation in pilots for new ways of providing care, as well as adapting to feedback and suggestions from patients. A Training Practice The practice participates in the advanced training of fully qualified doctors who have chosen to specialise in general practice. The additional Doctors work at the practice for up to one year and share all aspects of medical care with the partners and principal GPs. During their time here, we hope you will view them as we do, an integral part of our team. We also occasionally have student doctors and nurses attached to the practice. You will be advised accordingly, and if you wish to see the doctor or nurse alone, your wishes will be respected. Home Visits In order to provide a good service to all our patients we request that home visits are only requested by patients who are too ill to come to the surgery. Routine Home Visits are usually in the afternoon, after GPs have finished morning surgery. To request a Home Visit please call the Surgery on (0161) 432 0671, before 10 am, Monday to Friday, except in an emergency.

Nurse Practitioners / NP) Carole Turner Pam Mitchell Rachel Bailey Debbie Kent Sue Jones [NP] Sue Kennedy-Owens [NP] Practice Roles Nurse Practitioners – The Nurse Practitioner’s role is seeing patients with minor illness or injury as referred by Doctors and Triage Nurses. Duties Include: Giving advice on diet, exercise and smoking – potentially including prescribing nicotine replacement therapy if appropriate, provision of family planning advice including the prescribing of oral contraceptives, injectable contraceptives, diaphragms and emergency contraception. Practice Nurse – Duties include but are not limited to: Child Health & Immunisations, Travel Immunisations, Cervical Cytology, Women’s Health, Telephone Triage, Asthma & Diabetes, liaise with GPs, Health Visitors, District Nurses, Health Care Assistants. District Nurses – The District Nurse team provides a wide range of nursing services to those who are housebound and to people whose care can not be delivered in a more appropriate place. Their services range from simple tasks such as injections, eye drops and venepuncture to complex wound care and terminal care packages. Treatment Room Nurse – The Treatment Room Nurses provide services from 8:30am to 12 noon each weekday. The treatment room caters for those patients who are able to attend the centre for their care and includes many of the same services mentioned under the District Nursing role description. Health Care Assistants – Main duties include: Phlebotomy (Bloods), New Patient Checks, Performing ECGs as required by GPs. Diabetic Nurse Specialist – The management of diabetic patient care. In the absence of a specialist nurse, this role will be covered by GPs and Practice Nurses. Health Visitors – The Health Visitors cover the Health and Development Surveillance of 0 – 5 year olds on GP’s Practice Patients. Administrative Staff – Our team of receptionists, secretaries, general administrative and computer staff will be your link with the medical staff at the surgery. Management – The main duties of the practice managers include: Ensuring delivery and provision of services are met; Dealing with all non-clinical aspects of the practice’s organisation.

Practice Facilities Results General Facilities Treatment Room; Physiotherapy Room; 17 Consulting Rooms, male and female toilets, waiting room, Clinic Clerk, Baby Room and a Baby Changing facility in the Disabled Toilets. Disabled Facilities Patients will find the design of the improved premises includes a ramp leading to the main entrance of the building, a disabled parking lot, and a disabled toilet easily accessible from the waiting room. Please ask for further assistance if necessary. As there is only one disabled parking lot – the use is only for disabled patients. Toilets Male and Female toilets are opposite reception and a disabled toilet is based near the Practice entrance on the ground floor. On-Site Pharmacy There is a private pharmacy situated in the same building as the practice – it is not owned or managed by Heaton Moor Medical Centre. It offers the same services as any other pharmacy as well as being very convenient for prescription collection. Pram Store There is a covered Pram Store located next to Reception Results When you see a member of the primary care team at the surgery you may have bloods, urine, x-ray and other tests. Before you leave you will be given an indication of when to contact the surgery for the results of these tests. Please telephone for these results after 2pm. Please note that the receptionists cannot give out results unless a Doctor has seen the results first.

Community / Other Services at Heaton Moor: REPEAT PRESCRIPTIONS Unfortunately repeat prescriptions cannot be requested by telephone. They can be accepted in writing either by post, fax, on 0161 442 1025, online or by hand delivering into the prescription box in Reception. Prescriptions will be ready for collection in 48 hours.
