RESTRAINT DOCUMENTATION Please see Patient Care Policy R-2 The Restraint Documentation screens have been updated to reflect recent policy changes for ordering and caring for patients in restraints. All queries on this screen are required. Notice that documentation is now required hourly rather than q 15 minutes. The only information that will pull forward to subsequent documentations is that of the Current Physician Order.

The queries and F9 look-ups on the restraint orders will match those on the Restraint Documentation screen. Please review the order prior to charting to ensure consistency. RESTRAINT DOCUMENTATION Each order for restraints or seclusion for the management of violent or self-destructive behavior is limited to: 4 hours for adults 18 years of age or older up to a total of 24 hours; 2 hours for children and adolescents 9 to 17 years of age up to a total of 24 hours; and 1 hour for children under 9 years of age up to a total of 24 hours.

RESTRAINT DOCUMENTATION Note that you now have the option of documenting q 15 minute observation or continuous observation. A trained Registered Nurse may now complete and document the required 1-hour face-to-face medical and behavioral evaluation within an hour of the initiation of restraints or seclusion for a violent and self-destructive patient. This means that an RN must complete the initial restraint documentation. CNA’s may complete subsequent documentation. When a Y in placed in the Meets Release Criteria query, you will be required to enter a STOP Date and Time and the Debrief Summary screen will launch.
