Service Fee 6.5% of Premium billed in January NO added fees – Claims Management Cost (TPA) – On Line Claims and document access 24/7 – Loss Control – AWC

PREMIUM REFUNDING POLICY IN POSITIVE YEARS Service Fee of 6.5 % refunded annually First Performance Refund at 46 months 50% of performance refund retained as member equity until sufficient reserves are achieved Must Contribute to receive Performance based refund No penalty for poor performance

REASSESSMENT In The Event of a Negative Year Every member contributes Individual reassessment based on % of total contribution
