Fond du Lac CHIP Process Kya Schnettler, CHES Fond du Lac Family YMCA

How it all began…  Health Assessments Task Force was formed through CHIP process  Health Department, Agensian Healthcare, United Way  Goals 1. Provide quantitative and qualitative health data to identify, prioritize and monitor health needs of county 2. Develop coordinated and collaborative action plan 3. Engage community leaders to commit resources toward health improvement 4. Help Fond du Lac County residents improve their health

Methodology  Questionnaire  Community members and Stakeholders  24 health issues created based on Healthy People 2020  Respondents rated each issue on 4-point scale from “not a problem” to “major problem”  An additional open-ended question about the most pressing health issue was asked of stakeholders before the 24 health items to obtain a top-of mind answer

 2011 – health priorities identified  3 subgroups of concern created  general health  healthcare access and affordability  injury and violence prevention

4 Priorities for Fond du Lac County  Increase dental care access for children and adults with Medicaid.  Improve mental health access to enhance the quality of life of Fond du Lac County residents.  Decrease binge drinking and over- consumption of alcohol among youth and adults.  Increase the number of Fond du Lac County residents who are at a healthy weight for their height.

Formation of Living Well Coalition  2 obesity coalitions were active  School district aimed at healthy kids/families  Area businesses  Many great initiatives - very little coordination  1-year process to examine strategy  Transform WI Grant was written to hire coordinator of obesity efforts

After Transform WI grant  Agnesian HealthCare  Took financial responsibility for this position  single largest provider of health care in Fond du Lac County  Aligns with goals of preventative health and wellness versus curative health care.  Agnesian HealthCare is serious about improving community health and understands the importance of reducing the number of obese citizens. This funding is core to their mission.

January 2013  Community Wellness Coordinator – hired and housed at FDL Family YMCA  Living Well Coalition  Meets monthly with stakeholders including:  Health Department  YMCA  Agnesian HealthCare  Aurora Healthcare  FDL School District administrator  FDL School District Nurses  Parents  Local Business Representatives  FABOH  Local Gym Owner  Senior Center Coordinator

Current Initiatives Y5210 (YMCA, Agnesian, local schools, police) Farmer Market (YMCA, Agnesian, Fond du Lac Downtown Partnership)

Current Initiatives Eat Smart (YMCA, Agnesian, Local Businesses, Restaurant Association) Bike to Work/School Month – May (School district, FABOH, Police department, Agnesian)
