Fingerprint clearance Fingerprint clearance If you have been fingerprinted and the TEACH site does not indicate your prints have been received, you MUST file an OSPRA 104 form with the NYC Department of Education. This will request that your fingerprint clearance will be forwarded to the NYS Department of Education. The OSPRA 104 form can be downloaded from the main page of TEACH.
