Aims of this session 2 To identify reportable incidents in practice To define accident, incident and near miss To discuss the processes for reporting of incidents in practice

Key concepts An undesired event giving rise to death, ill health, injury, damage or other loss Accident Event giving rise to an accident or having the potential to lead to an accident Incident An incident where no ill health, injury, damage or other loss occurs Near miss 3

Why do we report? To prevent recurrence by: –Collecting data –Ensuring Heads of Departments are aware and can investigate properly –Panning follow-up activities which tackle the cause/ of the incident 4

How do we report? Give as much detail as you can Be factual – Do not speculate Be clear – Do not be vague 5 Mrs X slipped and fractured her hip on a wet floor outside reception Mrs X was found on the floor in the waiting area. She said that she had slipped on something but the floor was dry. Her right leg was shortened and rotated and a fractured hip was suspected

Reporting process Incident involving student Contact Placement Educator and Academic Tutor Dual reporting (organisational and S24 forms) Copies sent to Faculty Health and Safety Officer 9

Something is happened… What do I do? 1 Student involved in (includes witnessing) an accident/incident/near miss 2 Contact Locality Visitor/PALL and report incident 3 Gather and record evidence from people involved 4 Ensure the basic facts are recorded on both organisational and S24 forms 5 Ensure a copy is forwarded to Locality Visitor/PALL 6 Follow-up action agreed and record kept in student’s file 7 Students should NEVER submit statements without consultation with Faculty 10
