SA12-PE-04 Purpose and Scope The purpose of this self assessment is to conduct a multi-discipline team review (deep dive) of the Feedwater System following Recommendation #8 from INPO IER Level , Scram Analysis. This evaluation will be performed to validate the effectiveness of station processes to identify, track, and resolve off-normal or low margin conditions in the Feedwater System using the attributes listed below.

INPO Event Report, Level 2, Scram Analysis Recommendation #8 Conduct a multidiscipline team review (deep dive) of at least two systems or components that have been identified as significant contributors to plant scrams in this scram analysis or by station- specific analyses. Attachment 5, Technical Data Analysis, provides a list of scram-significant systems for each NSSS type to assist in determining scram-risk-significant systems. These evaluations will be performed to validate the effectiveness of station processes to identify, track, and resolve off-normal or low margin conditions.

Performance Objective 1 Determine whether or not the Feedwater system is or is not being operated as designed. Include current configurations, operator workarounds and burdens, disabled annunciators/alarms, and controls being operated in manual (vs. automatic).

Performance Objective 2 Determine if deficiencies exist in the Feedwater equipment, to include corrective and deficient critical and noncritical backlogs, which lower system reliability.

Performance Objective 3 Verify relevant internal and industry OE and vendor recommendations are reviewed for applicability for the FW system.

Performance Objective 4 Verify FW design changes applicable to off-normal or low margin conditions are addressed.

Performance Objective 5 Verify preventive maintenance work not excessively deferred for FW.

Performance Objective 6 Verify previous corrective actions to resolve Feedwater system and component vulnerabilities to scrams and transients have been effective.

Performance Objective 7 Verify that identified Feedwater issues are appropriately captured in system and component health reports and in the corrective action program
