Effective Use of the PSAP Screening Tool within a Community Health Program Model

EIPP Eligibility  Eligibility for EIPP is determined before a home visit is conducted.  Specific eligibility criteria  How eligibility screening is conducted is not standardized

EIPP Initial Assessment Tool: CHA  At the initial home visit, a comprehensive health assessment (CHA) is conducted.  A CHA is a health assessment in the broadest sense of the word encompassing 14 key assessment areas (KAA’s). KAA’s 1. Access & Utilization of Care 2. Women’s Health 3. Oral Health 4. Nutrition 5. Breastfeeding 6. Physical Activity 7. Cognitive & Perceptual 8. Environmental Health & Safety 9. ATOD 10. Violence 11. Emotional Health 12. Healthy Parenting 13. Neonatal & Developmental Assessment 14. Medical History & Physical Assessment

Adapted the PSAP Tool for EIPP  The PSAP Screening Tool was separated out into its three topic areas  The three PSAP topic area questions were then incorporated into the EIPP CHA KAA’s

Alcohol, Tobacco, & Other Drugs (ATOD)  At intake, 21% (423/2062) of EIPP Participants reported Tobacco Use  At intake, 13% (262/2062) of EIPP Participants reported Substance Abuse in their home  On the initial CHA, 34% (702/2062) of EIPP Participants had a positive screen

Violence  At intake, 11% (226/2062) of EIPP Participants reported violence in their home  On initial CHA, 32% (662/2062) had a positive IPV screen with violence occurring within the previous year

Emotional Health  At intake, 42% (868/2062) EIPP Participants reported a history of depression including post partum depression  On initial CHA, 57% (1174/2062) were assessed to have a positive emotional health screen of 1 or higher with a history of mental health concerns

Conclusion  Use of the PSAP tool improves identification  Highest increase in identification from the violence screen  Ease of use reported by EIPP Staff
