Throw away all old forms!!! Do not use evaluation forms on the Smart Steps CD. Each class has a unique identifier (number) Discard anything that has an old class number Only use the new envelopes to mail the evaluations Use the new attendance forms

Send in evaluations EVERY WEEK Make copy of attendance form each week and send in with evaluations. Ideally you keep the original attendance form through to the booster session and mark off who came to the booster session Make sure people write first and last names PLEASE put the names and ages on the Nursery and Children attendance roles
