What is our ILO? What is an MUI?

Intended Learning Objective =  Understanding what constitutes an MUI  The difference between an MUI and UI  How to report  MUI categories

 MUI or Major Unusual Incident is an alleged, suspected, or actual occurrence of an incident when there is reason to believe the health and safety of an individual may be adversely affected and the individual may be placed at a reasonable risk of harm.

Is this an MUI?

MUI Scenarios~ Is this an MUI? 1. Staff left individual in the van for approximately 3-5 minutes while they went into the bank to get individuals funds for a shopping trip. Individual has 24/7 supervision and is to be within eye sight when in the community. Provider reports that staff could see the individual from inside the bank. Y or N 2. An individual went into the break room in his wheelchair and stopped at the door blocking it so others were unable to get by. Staff yells across the room “stop blocking the door or I will come over there and move you.” Y or N 3. The waiver provider and individual are watching a porn movie. The individual turns to see staff masturbating. Y or N 4. Staff person working 3 rd shift hears a noise in one of the bedrooms. Staff person finds individual lying on the floor beside the bed and is bleeding. Individual receives 8 stitches at the local hospital. Y or N 1. MUI ? CIRCLE - ( YES) Risk 2. MUI ? CIRCLE - (NO) 3. MUI ? CIRCLE - ( YES ) 4. MUI ? CIRCLE - ( YES ) CATEGORY – Neglect CATEGORY – Verbal Abuse CATEGORY – Sexual Abuse CATEGORY – Unknown Injury

5. When staff went in to check on an individual who was watching TV, they were gone. Staff started searching and found him 10 minutes later on the swing in the backyard... Y or N 10. It was discovered that $80 was missing from three individual’s lock box. Individuals live in an IO Waiver home. Y or N 15. A concerned citizen calls the county board and reports that they witnessed a man hitting an individual in front of a house in their neighborhood. The victim appeared to be disabled and may be eligible for MR/DD services. The citizen gave the board the address and a description of the two men. Y or N 13. Nurse gave an individual the medication that belonged to a different individual. After receiving the wrong medication the individual was taken to the ER and his stomach was pumped. Y or N 5. MUI ? Are not consistent w/ daily operations Or care/habilitation of that person Unusual Incidents may be events involving a person w/ developmental disability which….. Medication errors where there is not a reasonable risk of harm; falls; Minor peer-to-peer incidents; natural disasters, fire, mechanical failure Unusual Incidents are anything that is not an MUI……

An MUI contact person is identified w/ each County Board Take immediate action to protect individual from harm Report incidents to board and all vested parties [including guardians ] identified in ISP Implement preventative measures which may be identified. MUI Reporting ABUSE, NEGLECT, MISAPPROPRIATION, EXPLOITATION, AND SUSPICIOUS DEATH must be reported w/in 4 hours of the incident! All others 24hrs w/in the incident. (w/ends on Monday- unless one of the above)

JEOPARDY  Break into two teams.  One point awarded for each correct response.  Remember to respond “What is”…  Team w/ most points wins! Any incident that results in the individual being charged, incarcerated, or arrested. Person who is required to report has reason to believe an individual has suffered a wound, injury, abuse, or neglect and such person does not immediately report information to proper authority Physical attempt by individual results in ER tx., observation, or hospital admission Death of an individual An injury from a known cause that is not considered abuse or neglect, which requires cast, 5 or more sutures, dental injuries Unlawful or improper act of using an individual or their resources for monetary/personal benefit or gain JEOPARDY What is Attempted Suicide What is Death What is Exploitation What is Failure to Report What is Known Injury What is Law Enforcement

Incident where emergency medical intervention is required to save life Depriving, defrauding, or otherwise obtaining real or personal property of individual Incident that is not considered neglect, and individual cannot be located for a period of time longer than specified in ISP There is a duty to do so, failing to provide individual w/ any tx., care, goods, supervision, or services necessary to maintain health/safety Employee engaging in consensual sexual conduct/contact w/ individual who is not the employee’s spouse, and for who employee was employed for JEOPARDY Clockwise: What is Medical Emergency; What is Misappropriation; What is Missing Individual; What is Neglect; What is Prohibited Sexual Relations

Violation of Individuals Rights Use of any aversive strategy or intervention w/out approval or consent of ISP team Injury of an unknown cause that is not considered abuse/neglect, which requires tx. only a physician can provide Hospital admission that is not scheduled unless the admission is due to a condition that is specified in the ISP JEOPARDY What is Rights Code Violation What is Unapproved Behavior Support What is Unknown Injury What is Unscheduled Hospital Admission
