Overall aim of the Personalisation Strategy To establish a people powered approach to commissioning that shifts the culture and practice of care to be better co-ordinated and person-centred.

The Self Care Strategy Aim To work towards a future state in which patients are empowered to self- care, taking responsibility for their own health and wellbeing, and where health and social care professionals are equipped with the tools, techniques and resources to support patients on this journey.
