Living Will Expresses your wish toward end-of- life decisions involving the use of “extraordinary measures” Not a substitute for your doctor discussing your wishes with you in person.

Living Will When will it matter? If you have an “incurable injury, disease or illness” and life prolonging procedures would serve only to artificially prolong the dying process; and Your doctor confirms this in writing.

Appointment of Health Care Representative a.k.a., “Health Care Power of Attorney” Allows Representative to make health care decisions if the patient can no longer do so.
