Welcome to Open Enrollment! Elections made during open enrollment will become effective on January 1, ECC offers you and your eligible family members a comprehensive and valuable benefits program. We encourage you to take the time to educate yourself about your options and choose the best coverage for you and your family.

Who is Eligible? Full-time employees who work 40 hours or more per week. Medical, Dental, Vision, Voluntary Life, Disability Spouses and Dependents of eligible employees. Medical, Dental, Vision, Voluntary Life

When to Enroll The open enrollment period runs from Friday, November 11, 2011 through Friday, December 2, The benefits you elect during open enrollment will be effective from January 1, 2012 through December 31, 2012.

How to Make Changes Unless you have a qualified change in status, you cannot make changes to the benefits you elect until the next open enrollment period. Qualified changes in status include: Marriage Divorce Legal separation Status change Birth or adoption of a child Change in child’s dependent status Death of a spouse, child or other qualifying dependent Change in residence due to an employment transfer for you, your spouse or domestic partner Commencement or termination of adoption proceedings Change in spouse’s or domestic partner’s benefits or employment status

What’s New for 2012 ?

Medical and Prescription Drugs Several changes have been implemented to the medical and prescription drug benefits for the upcoming plan year.  GHP will be your new medical insurance carrier  ECC employees will still have three plan designs to choose from Base Plan – $500 deductible with 80% coinsurance Buy Up Plan - $0 deductible with 100% coinsurance H.S.A. Plan - $2500 deductible with 100% coinsurance  New Copay amounts for the Base and Buy Up plans  New Deductible amount on the H.S.A. plan  Some prescriptions may fall into a different tier with GHP than with UHC

Basic Life Insurance ECC provides full-time employees with $50,000 worth of group life insurance, and pays the full cost of this benefit.  Contact Human Resources to update your beneficiary information or complete a change form today…

Voluntary Life Insurance Employees who want to supplement their group life insurance benefits may purchase additional coverage. When you enroll yourself and/or dependents, you pay the full cost through payroll deductions. You can purchase coverage on yourself and your spouse and your child. Contact Human Resources to apply for supplemental life insurance or complete a change form & evidence of insurability form today…

Health Savings Accounts (HSAs) If you participate in the high deductible health plan (HDHP), you can set aside money in a Health Savings Account (HSA) before taxes are deducted to pay for eligible medical, dental and vision expenses.  An HSA is similar to a FSA in that you are eligible to pay for health care expenses with pre-tax dollars.  Advantages of an HSA Unused money is not forfeited at the end of the year and is carried forward. The account is yours to keep so that you can take it with you if you change jobs or retire. If you have any money remaining after your retirement, you may withdraw it as cash. H.S.A. dollars can be used to pay qualified medical expenses for yourself and family members.

Health Savings Accounts (HSAs)  The maximum amount that you can contribute to an HSA in 2012 is $3,100 for individual coverage and $6,250 for family coverage.  If you reach age 55 or older before the end of the year, you may make additional “catch-up” contributions of $1,000

Forms to be Completed GHP medical enrollment form – everyone must complete. Reliance dental enrollment form – only if you wish to add or delete family members. If no form is completed, your enrollment will stay the same as with UHC. Reliance supplemental life form and evidence of insurability form to change your Voluntary Life insurance for yourself or family members. If no form is completed, your coverage will remain the same. Beneficiary form – only if you would like to make a change. Completed enrollment forms must be turned in to H.R. by Monday, December 5, 2011.

What to expect New GHP medical I.D. cards will be mailed to your home address. You should receive them prior to January 1 st. New Reliance dental I.D. cards will be mailed to your home address. You should receive them prior to January 1 st.
