Nurse’s Notes Dates to Remember: October 6 th & 8 th – Dr. Tomley, the school doctor, will be at Shaffer to do the mandated school physical for K and 6 th grades. October 19 th – Don’t miss this opportunity to have your child’s mandated dental exam done by the Smile Dental Mobile. This is a great program that provides cleaning, x-rays and sealants for students, age appropriate. If you did not receive the required form for the school physical exam or the dental mobile exam, please call or visit Nurse Walter. Immunizations: Kindergarten students are required to have in writing from the doctor verification of: 4 DTP immunizations. The 4 th immunization after the 4 th birthday. 3 Polio 3 Properly spaced Hepatitis B 2 Measles, Mumps and Rubella 2 Varicella (chicken pox) or proof of the disease The mandated immunizations are given free of charge at the Allegheny County Health Dept. – M, T, Th, F from 9am-4pm and Wednesday 1pm-8pm. Students not up-to-date on the above immunizations are considered provisional and will be excluded from school if immunizations are not complete.
