Open Enrollment Basics

Your annual chance to review your benefit elections November 1 – November 30 of each year Most employee changes made on-line at: Same sex domestic partner applications done by paper – not available online at this time Additional information on Employee Benefits website at:

Human Resources Office of What benefits can I change? Add or cancel Medical, or change plan options Add or cancel Dental, or change plan options Add, change, or cancel your dependents

Human Resources Office of What benefits can I change? Elect to participate in the Health Care and /or Dependent Care Flexible Spending Account New election required each year Elect to participate in Voluntary Long Term Disability coverage for Civil Service and Represented employees, or change your LTD benefit amount

You don’t need to go on-line and change your elections unless you’re an FSA participant and want to continue for 2007 if you only want to change clinics, call your health plan directly 2006 is a voluntary open enrollment – no changes required

UPlan Pharmacy Program

Human Resources Office of Does the UPlan Pharmacy Program have any changes for 2007? No, RxAmerica will continue as the pharmacy benefit manager for all UPlan Medical options in January, Rx copays will remain the same for 2007 for all plans except the Medica HRA/HSA’s Generic Plus $10 Formulary Brand $20 Non- Formulary $35

If you or your doctor request that you take a brand drug when a chemically equivalent generic is available, you pay the Generic Plus copay + the difference between the cost of the generic and brand drugs, unless you have a prior authorization in place.

Civil Service / Represented Employees may enroll in Long Term Disability coverage during open enrollment Pre-existing Condition Limit: Disabilities related to medical issues occurring during 2006 will not be covered for 24 months

New STD enrollments or increases in coverage will require proof of good health If interested, contact Employee Benefits for a paper application, and instructions for providing health information Changes effective 1/1/07 or later

Changes from Minnesota Life to The Standard effective 1/1/2007 All coverage automatically transfers Additional information will be sent to covered Faculty and P&A members

Life & Long Term Care Insurance

Long Term Care Insurance Coverage will change from CNA to John Hancock, effective 4/1/2007 New program will include:  Improved benefits  Lower rates for many participants  Expanded eligibility  Guaranteed issue coverage for all active employees Detailed information will be provided beginning in January

Human Resources Office of What’s new for UPlan Wellness? 10,000 Steps Available to all U of M employees  + Spouses/SSDPs and Early Retirees on UPlan  Enroll by 12/31/2006  Complete in 2006 or 2007 Free pedometer, on-line step tracking Motivational messages Employees enrolled in the UPlan who have not received a second $65 reward can earn their reward for completing 10,000 Steps
