Open Enrollment 9/15/2010 – 10/15/2010 What you can do during Open/Annual Enrollment Enroll in Health Coverage Enroll dependent children up to age 26 for health & dental benefits. You must provide proof of dependent eligibility. Enroll in Flexible Spending Accounts Add, cancel or transfer Dental options Apply for or decrease Optional Term or Universal Life Coverage Apply for Long Term Disability Enroll in Optional Special Accident NO GUARANTEE OF FUTURE OPEN ENROLLMENTS
Open Enrollment 9/15/2010 – 10/15/2010 What you MUST do during Open Enrollment Select a Health Option – Partnership or Standard Select an Insurance Carrier – Blue Cross or Cigna Select the Premium Tier Complete the new enrollment form and turn in to TTU HR no later than 4:30 pm 10/15/2010. If you are currently enrolled in the State’s Health Plan – you must complete a new enrollment form OR you will be enrolled in the Standard PPO with the lowest cost carrier. No changes until next Annual Enrollment
Plans to Choose From Effective January 2011, we have two new health options. Partnership PPO Standard PPO Cover the same services, treatments and products. Your costs, including premiums, deductibles, co-pays, co-insurance and out-of- pocket maximums will always be less with the Partnership PPO

Active Employee – NOT Currently Enrolled If you want to enroll in the health plan, you must complete the enrollment form. If enrolling dependents, you will need to provide proof of eligibility. Must complete all information Sign and date form Run a copy for your records, bring both original & copy to HR. HR will date stamp both for your records. HR cannot run copies for you. DEADLINE: 4:30 PM OCTOBER 15, 2010 – NO EXCEPTIONS!

Flexible Benefits Plan Open Enrollment 9/15/2010 – 10/15/2010 Period of Coverage 1/1/2011 – 12/31/2011 Medical & Dental Premiums will automatically be paid on a pre-tax basis. If you do not wish to pay premiums on a pre-tax basis – you MUST complete the WAIVER box at the bottom of the Flexible enrollment form. You must complete the enrollment form if you want to tax shelter any medical or dependent care expense other than the premiums. Effective 1/1/2011, certain Over-the-Counter drugs and medicines will no longer be eligible for reimbursement without a prescription from your doctor.
