The Diabetes Prevention (DP) Demonstration Project  8 Core elements - Intensive Intervention – “the what” Screen and Identify eligible individuals with Pre-Diabetes Recruit individuals with Pre-Diabetes to participate Initial physical exam and baseline measures Education on diabetes prevention using DPP 16-session curriculum
