A shift towards prevention No Concerns identified at 27 Month Child Health Review

Aim of the Improvement Work To maximise global development and wellbeing outcomes for children at months of age by earlier detection and prevention of areas of concern. Aim 95% of children aged months of the test population designated by the Health Visitor caseload in the Coatbridge locality will have a review completed by 31 March 2015 and provided with interventions around areas of concern and anticipatory guidance to prevent areas of concern developing at a later stage.

How we will change to improve to reach this Improvement Goal

Health Visitors Voice Health visitors leading the way! Have been able to rapidly apply learning from tests of change Developed their own parent feedback form and anticipatory guidance checklist based on their learning while carrying out the test. We have a passion and want to get it right for every child (GIRFEC), were in a team and can respond as a team. We have drive and ambition to make things better.

Our Journey so far P DS A 1) Started at the end of July 2) Saturday clinic with 45 minute appointments 3) We tested attendance on a Saturday of families that had been identified as HPI of ‘additional’. Anticipatory Guidance The team started using the checklist on 05/09/2014. The feedback on the checklist has been very positive from the team. They have found it very useful, especially when filling out the wellbeing assessment.

Feedback from Parents Q1 I have found this assessment useful? Q2 I found the information given during the assessment useful? Q3 I would recommend this contact with a health visitor be available to other parents? 21 Parental Review Questionnaires received as of 07/10/14. All have answered Yes so far to these questions. We are learning that parents value the strengthening relationship with health visitor and reassurance given, not just the assessment itself.

+ Comments by Parents After my first baby didn’t have any further assessment then just before school found out she needed speech therapy. I think early assessment like this is excellent for picking up any issues. Very informative, particularly having older child with autism. Good to know my child is doing well. Found this to be very useful and beneficial. It provides the opportunity to have reassurance on my daughters developmental stages. Many thanks. I found the assessment helpful, to know what stage my child was at for her age.
