What does the Act say? Local authorities MUST: Ensure the integration of care and support provision with health provision and health-related provision Co-operate with each of its relevant partners, and each relevant partner must co-operate with the authority, in the exercise of their functions relating to adults and carers Co-operate where this is needed in the case of specific individuals who have care and support needs 6

Better Care Closer to Home Strategy (BCCH)  Key joint strategy  Create holistic integrated community services in the community to maintain and improve people’s health and wellbeing  Shift in settings of care from hospital to community  Funding from Better Care Fund will deliver parts of the Strategy

Better Care Fund (BCF)  The BCF is a pooled budget that shifts resources into social care and community health services for the benefit of the NHS and local government  Requirement to measure impact on reducing avoidable hospital admissions and reduce length of stay

Outcome Based Commissioning (OBC)  Initiative of Richmond Clinical Commissioning Group (CCG) and Richmond Council  Aims to transform the delivery of community services based around outcomes that matter most to local people  This model is still in development and some services will be delivered in this way from April 2015.
