UPS Bow Tie Fluids Monitoring This Upstream Process Safety Near Miss highlights the need to fully comply with set procedures as this incident had the potential to escalate into a well control scenario. Actions Reiterated the requirement for the communications matrix to be followed as per procedure and full compliance with agreed reporting chain. Ensured regular discussions with crews to confirm monitoring procedures and communications matrix understood and followed. Ensured a copy of the above matrix is displayed prominently in the SLS unit. Reviewed Upstream Process Safety BowTies and started implementation of checklists at the rig-site.

NEW ORGANISATION Step Change in Safety (2014) Limited SCiS is a company limited by shares 100% owned by OGUK Trade, assets and liabilities of SCiS to be transferred to new company limited by guarantee Articles of Association of new company will reflect principles of existing SCiS constitution Step Change in Safety Limited A company limited by guarantee has members rather than shareholders- Member owned – no equity stake but limited by guarantee Liability of members will be limited to £1 Transaction to be effective on 1st January 2015

NEW ORGANISATION Board requirements:- Co-chairs Deputy co-chairs Union Member HSE Member Team Leader Board will be responsible for fiduciary and governance issues. Leadership Team will continue to be responsible for strategy and operations. Action:- Nominations required for deputy co-chairs to be received by 24 th December.

Standing Agenda Safety Moment or Topic to be shared around SCLT nominated by support team unless someone volunteers Doesn’t have to be an incident report, could be something more general that the SCLT would benefit from hearing about 2 work stream reports per meeting (by exception if a decision is required - input to be available at least 48hrs prior to SCLT for distribution) Focus on progress updates against rolling plan ACTION: each work group to develop and share a rolling work plan, highlighting any potential key decision points or milestones, by end 1 st Q 2015 Peer review & challenge against the 3 key themes of simplification, engagement and sustaining SCLT Engagement in work groups Broad discussion to take cognisance of weak signals and emerging issues (regulator, workforce, union, employer, TA input) A check in in case there is an emerging issue we should take cognisance of Attendance – If SCLT member cannot attend, 2IC should be nominated individual.

ASSET INTEGRITY MAH Management Capability Terms of Reference Purpose & objectivesTo prevent Major Accidents through the education of the Industry’s workforce in “Major Accident Hazard(MAH) Management” and “Process Safety Management (PSM)”. To develop a training programme and materials required to embed a process safety culture throughout the Industry so that everyone understands MAH / PSM and the part they play in preventing a major accident. To ensure that the training is implemented effectively across the industry to develop a more informed workforce.

ASSET INTEGRITY HCR / MAH Implementation Workgroup Terms of Reference Purpose & objectivesPurpose: support hydrocarbon release reduction efforts and improve asset integrity in UKCS Objective: 1)Develop a methodology for assessing individual companies HCR / MAH plans, providing objective feedback on the plans and promoting cross company best practice through mentoring. 1)Develop a set of performance indicators for HCR prevention / MAHs that can be promoted across the industry.
