HIV care & Treatment service Chronic care Correct & consistent documentation Quality service Better patient outcome Data quality attributes checked: Data accuracy Availability & use of the data tools Data completeness Consistency of data

DQA done Gaps Identified Workplan on bridging the gaps Workplan implemen ted Data Quality Assessment tool used..\Data tools\Goldstar DQA tool.xlsxSlide 4..\Data tools\Goldstar DQA tool.xlsxSlide 4 Used once every 3 months per facility Better results expected with each DQA Methodology

Data analysis Regional scores per the data attributes

Cont.……… Data analysis Improvement in all the attributes. Consistency of the exercise may result to better results over time

Findings General improvement across all the data attributes Tools availability scored the highest since it’s largely program driven Data consistency which is largely provider driven performed the least Despite adoption of a similar model across all the regions, the scores were different due to: – Program length in the region – Number of patients in the program

The model is applicable for any program The tool used is an easy to use excel sheet Incorporates a workplan on how to address the gaps Improvements recorded over time acts as a motivation to the health care worker. Advantages

Financial data given more attention than Program data in private facilities High staff turnover had an impact on the workplan implementation Challenges

Frequent audits help in identifying and addressing the gaps early It’s Important to have the private providers appreciate the importance of program data Quality data is key in chronic care for effective determination of patient outcomes Conclusion

Thank You
