Background Integration of Health and Social Care Improving Local Services 2 Projects: –Lerwick –North Isles (Yell and Unst)

Lerwick Localities Project Aim: To determine the current pathways accessed by service users across Health and Social Care in the Lerwick area. Focus: –Admission Avoidance –Early supported discharge

North Isles Localities Project Aim: A review of the care model delivered to services users accessing Care Homes in the Northern Isles. Focus: –Assessments undertaken –Care provided –Discharge arrangements

Where did we go? Community Teams Intermediate Care Team Hospital Primary Care

What did we do (Exploration)? Supported staff to develop high level process maps of their service area: –Showcase strengths –Highlight areas that require improvement –Identify gaps –What could be done differently
