Outcomes & Achievements 1.Awaiting discussions from Oxfordshire CCG as they are discussing Outcomes based commissioning. 2.Historically trialed POS / SKIPP 3.ANP / CNS review for the whole organisation - portfolio‘s for team 4.Commissioning income more stable from reporting and reconcilliation.

Challenges 1.Increased complexity of patients 2.Increased challenges working within community setting as less district nurses and carers 3.Role blur between community matrons & specialist nurses 4.IT challenges 5.Retention & recruitment to specialist posts, particularly band 7 posts 6.Commissioning changes across Oxfordshire has seen numerous changes 7.Little scope for development and expansion in current financial climate

New & Future Plans 1.Have introduced band 6 posts as increased resource and succession planning 2.Away days for whole team to support team development and resilience 3.Better liaison with Oxford Health 4.Wider Trust strategy for end of life care 5.Wider Trust and National work on resilience
