Why? General practice……..has shifted to demand-led, reactive health care provision,………. losing the value of population health care, wellbeing and empowerment Dr Charles Alessi, Reclaiming a population health perspective (2013)

Why not? Challenges facing primary care demand from patients - GP consultations + 75% lack time to reflect on how to provide and organise care more responsibilities – commissioning/reduce A&E attendances/admissions/integrate care for elderly

Inequalities Each CCG whilst carrying out its functions must have a regard to the need to reduce inequalities between patients with respect to their ability to access health services, and reduce inequalities between patients with respect to the outcomes achieved for them. (NHS England, 2013)

Inequalities Inequalities in access to GPs exist despite new contractual approaches Economically disadvantaged groups often have access to smaller number of GPs

Key challenges tobacco, alcohol BMI are leading risk factors for illness and disability

Opportunities Shift to CCGs – practices can influence how funds can be spent Registered list - take a population-based approach to planning and delivering care for registered patients Locality working (Gateshead) – leadership to shift emphasis from reactive to proactive – not just about practices – other partners – innovation – funding but more than £
