2. Order Forms  This is a typical order form for an American newspaper. An order form for a magazine would be very similar. Fill in the blanks assuming you’ll pay for the newspaper with your Visa Card account number

Order Forms Sample

Order Forms Task 1 Design a new order form according to the information Write an order form for an American monthly magazine which costs $12.00 per year. Assume that the magazine is called “Harper’s Magazine” and that it claims to be America’s best source of fact, thought, opinion, and humor. At the newsstand it costs $2.00 per month, so 50% can be saved on a year’s subscription. The magazine’s actual address should appear at the bottom of the order form. It is: Harper’s Subscription Department, P.O. Box 7511, Red Oak IA 51591, USA.

Order Forms Task 2  Write a new order form for a weekly magazine, which costs either $52.00 per year or $2.50 per week. Assume that the magazine is Time Magazine (you may be familiar with it). Make it clear that the customer saves 60% by ordering this magazine for a full year. One may pay by sending a check or billing it to a credit card. If they use a credit card, they should say whether it's Visa, Master Card, or American Express.

3. Registration Forms The Registration form is an official list or record containing a "summary" of related information about a person. Be sure to fill out all sections in full.

Registration Forms  Every student must register for classes each semester. Registration includes enrolling in classes, the telephone registration system, paying fees and other financial obligations, filing ( 提交 … 以备案 ) your current address with the Office of the Registrar ( 注册办公室 ), and providing other information forms.

Registration Forms When you register, you must:  Have a photo ID picture taken.  Submit a Statement of Legal Residence.  Return the completed Medical History form and the Insurance Information Request form. These forms are mailed to each new student by the Student Health Center.

Registration Forms Tips  Be sure to inform the Office of the Registrar of any change of your address, or your name. Keep your campus / local mailing address, your billing address and your permanent address up to date. Failure to file your current address can result in a delay of your registration  (These are usually things required in the U.S.).

4. An Application form for Admission  When you are applying to study abroad, one of the important things you need to do is to fill out the application for admission form. The form usually contains several parts: personal details, course details, arrival information, requests for accommodations, education and work experience, and foreign language proficiency, etc.  Get familiar with the following application form, and fill it in as a practice.

Please check it frequently.)

TO ALL CANDIDATES FOR ADMISSION  Probably no part of a college application is more talked about, misunderstood or feared than the form you are about to complete. For this reason we’d like to advise you of the following.  First, we are interested in the total person (not just the classroom part of you), and so we want to learn how you spend your time when you are out of school.  Second, we’d like to know about any exceptional skills or experience you have, both in and out of class. (We realize that the way you spend your time may be either limited or enhanced by the opportunities available, the need to work or frequent family moves.)

(continue) However you use your time, we are more interested in the depth of your commitment and achievement than in the sheer quantity of your activities. We appreciate candor rather than false modesty or bragging. Remember that the information you provide is only part of the information we will use to evaluate your application. So, be yourself and complete this form carefully and honestly.
