NDC Outcomes Improve how people rate their own health Decrease the proportion of adult population who smoke * Increase the uptake of physical exercise Reduce the number of children on the child protection register in the area  Reduced Teenage pregnancies/conceptions  Reduce the numbers of admissions to hospital  Reduce the level of alcohol misuse in the area  Reduce the level drug misuse in the area  Reduce the level of mental ill health  Increase the proportion of people who eat 5 portions of fruit and vegetables every day

Improving health – national performance indicators North Fulham NDC Rate of smoking (household survey) NDC England Rate of no physical activity for at least 20 minutes at a time (household survey) NDC5819 Health good or fairly good over last year (household survey) NDC England Notes National performance indicator data has been provided by the National Evaluation Team 1 General Household Survey 2000/1, 2002/3, 2004/5

Results : Smoking Population data ; Prevalence Smoking Outcome Measure To reduce the prevalence of smoking amongst adults to 18% by Year 10. Baseline Position – 31% NDC adults smoked ( Mori) Position at last PMF % NDC adults smoked( Mori) Latest Milestone – reduce adult smoking to 24% Latest Actual – 25% (National Evaluation Data 2006) Next Milestone – reduce adult smoking to 21%

Results : Smoking Service data & project data Service data week NDC quitters through smoking cessation service – 4 in Qtrs in Qtr 4 NDC telemarketing service begun in Sept 06 Project data –Number of NDC residents contacted through telemarketing 473 and 224 referrals made 64.2 white British etc PCTs are now moving to prevalence rates so there will be Borough comparators

Results Service Data : Child Protection Child Protection By year 10 reduce the number of children in the NDC area on the child protection register by 50% (a reduction of 8 children via the Children in Need Index) Baseline Position – 16 children on CPR Position at last PMF – 7 children Latest Milestone reduce children on CPR by 25% to 12 Latest Actual 4 children in 2006 (Children in Need Census 2006) Next Milestone – reduce the number of children on CPR by 37% to 10 **Reducing the gap : Concentrating on obtaining %’s of children % of Borough’s Children on CPR – NDC 0.35%
