Development of an Apple iPhone IOL Calculator Application for Cataract Surgery Rony Gelman, MD 1, Richard E. Purpose To demonstrate the feasibility of an intraocular lens (IOL) calculator application (commonly referred to as an “app”) for the Apple iPhone mobile device platform. To compare the results of the app against the IOLMaster reference standard for a set of test cases.

Methods The iPhone software development kit (Xcode version 3.2.5; Apple Inc., Cupertino, CA) was used to develop the IOL calculator app. The app was developed on an iMac (2.66 GHz Intel Core 2 Duo; 4 GB 1067 MHz DDR3; Apple Inc., Cupertino, CA) running Mac OS X (version ). The iPhone app was tested on the iOS Simulator (version 4.2) and an iPhone (model 3G S; 16GB memory; iOS version 4.2.1; Apple Inc., Cupertino, CA).

Methods - continued The IOL calculator app implements four standard intraocular lens calculations used for cataract surgery: – Hoffer Q 1-4 – Holladay 1 5 – SRK II 6,7 – SRK/T 8 The IOLMaster (version 5.4; Carl Zeiss Meditec AG, Jena, Germany) was used as a reference standard.

Methods - continued 10 patients (20 eyes) were selected from a database of patients that underwent cataract surgery with biometry data obtained from an IOLMaster. 4 intraocular lens models were selected per eye. For each lens, the 4 IOL formulas were tabulated for 7 refractive targets. The combinations of eyes, lenses, IOL formulas, and refractive targets yielded 2,240 intraocular lens power data points for comparison between the iPhone IOL calculator app and the IOLMaster.

Results The IOL calculator app was successfully implemented using the stated Apple hardware and software (figure 1,2). Characteristics of the 10 test cases (20 eyes) are listed in table 1: – Axial lengths ranged from mm. – Mean keratometry values ranged from D.

Results - continued All 2,240 (100%) IOL powers calculated by the iPhone IOL calculator app agreed with those derived from the IOLMaster.

Conclusion It is feasible to develop an intraocular lens calculator app for the Apple iPhone platform. The IOL calculator app showed agreement with the IOLMaster reference standard. Future work will include a larger test database, including more extreme ranges of axial lengths, mean keratometries, and target refractions, as well as tests of other IOL models.
