Second Screen (not shown) –Member Number : Enter your social security number –Zip Code: Enter your home address zip code –Click Submit Third Screen (not shown) –Follow instructions on the screen –User Name: Social security number – or change to what you want –Password: Follow instructions on the screen for setting your password –Confirm Password: Type password again –Security Question: Choose the question you want –Security Answer: Type your answer – case sensitive – Address: Must give a valid address –E-notify: Check mark the box if you want an sent to you when we receive a claim Please put your password in a location that you will be able to find it. PayFlex will not be able to see your password in the event you forget it or lose it. DePaul University Transportation Program Read the Provided Description and select your “Product Name” This example is for “Chicago Card Plus 30-Day Pass”

DePaul University Transportation Program Select “No, I need a card” or “Yes, I have a card” If you have a card, enter your card’s serial number and PIN. Otherwise, a new card will be sent to you and the CTA fee of $5 will be added to your order. Enter a 4 character alpha/numeric PIN for your new card.
