What Is Person Centered Planning? Person Centered Planning goes by many names – MAPS – PATHS – Essential Lifestyle Plans – Personal Futures – Individual Design Sessions You might have heard of some of them

What Is Person Centered Planning? Planning that puts the person at the centre of it Sounds simple? Let’s have a look at what life usually looks like when you have a disability....

Word Smart I like to listen when people talk I get annoyed when people use the wrong words I like to learn new words I like to read I can give people good directions I can remember names and dates and trivia.

Maths Smart I like doing Maths at school I can add numbers up without using pen and paper I like doing science experiments I like doing puzzles and brain teasers I like calculators and computers I like to know how things work

Picture Smart I like drawing better than writing I can find my way around new places I like to take things apart and put them back together I can read maps and pictures I like to build things I do drawings in my notebook

Body Smart I use my hands when I talk I can run or swim without getting tired I like to touch things I can ride a bike and I find it easy I can copy other people I find it hard to sit still for a long time

Music Smart I can play a musical instrument I listen to music alot I hum and sing alot I remember the melody of songs I know when music doesn’t sound right I can sing really well

People Smart I like to work in a group I can tell when my friends are sad I have more than 2 close friends I am good at helping my friends with their problems I am good at being a leader I understand other people’s feelings

Self Smart I like to be alone I have a collection of special things I remember my dreams I am very independent I can say how I feel I know what I am good at and what I am bad at

What SORT of SMART Now you know what sort of smart you are... How does this match with the job that you are doing now? How do the work experience placements students have done in the past match with their sort of SMART?

Transferable Skills Many skills that someone already has can be TRANSFERED into another area First you need to find out what a person can already DO Then you can MATCH those skills to tasks

Transferable Skills You can use a SKILLS INVENTORY to find out what people can already DO Then break it down into TRANSFERABLE SKILLS This is called TASK ANALYSIS

What is the person’s CAPACITY CAPACITY is what a person could do with the right SUPPORT SUPPORT means: – What does someone need to learn? – How does someone learn? – What other needs might someone have?

What WORKS? Finding out what WORKS helps you to figure out what SUPPORT a person needs It also helps to create a realistic picture of how a person interacts with the world around them By finding out what DOESN’T WORK you can ensure that a person’s first experiences of work are positive experiences
