(answers to questions 1, 2, 5)  A Quality Improvement Plan (QIP) (answers to questions 3, 4)  Report presented to the EVP/AVP/VP  At this point, discussions can be had about what improvements may be appropriate to submit into the ACC Master Plan process. 8/4/2015

What Happens Next? 8/4/2015 Five Fundamental Questions Answered by SSR/QIP, Years In years 2-5, they will submit an update of their QIP, including where they are in implementing their plan and the data for their measures showing movement one way or the other.
