Note: If there are pending investigations or substantiated findings noted above, detailed information, including evidence summary, hearing, or rebuttal statement, may only be obtained by calling Monday through Friday from 9:00 a.m. to 3:00 p.m. and speaking with a registry representative.

This information is released to DHHS, whom releases info to Organization, as applicable State hits can be released to the Organization. Organization will be notified if any National “hits” are found, cannot be released directly to the provider, but to the applicant. Applicant should discuss results with employer.
