CPR & AED STS 1/26/15

You get a dispatch call to McCarthy Quad and find a 21 y/o male lying supine on the grass. You arrive on scene. What is your first step?

1. CHECK AVPU. Patient is unresponsive. What do you do next?

2. CHECK PULSE. Where do you check? Carotid pulse. What is the normal pulse rate? There is no pulse. What do you do next? bpm

3. START COMPRESSIONS. What rate? How deep? What ratio? What should you look for while ventilating? What should your partner be doing? Between 95 and 105 per minute (to the beat of “Staying Alive”) 2 inches 30:2 Chest rise and fall. What if you don’t see it? Ensure adequate head tilt chin lift. Make sure breaths are around 1 sec each. Don’t ventilate too quickly. Grabbing the AED! (& cutting off patient’s shirt)

3A. VENTILATE. How do you ventilate? Use OPA/NPA unless contraindicated due to: How do you measure it? 2-person ventilating technique OPA: gag reflex Both: head/facial trauma Important: Make sure suctioning equipment is on hand and use as needed.

4. USE THE AED. What should you make sure of before you use it? No clothing. No water. Then just follow the instructions… While waiting for AED to charge, resume compressions. and IMPORTANT: make sure no one is touching the patient when AED says: “Do not touch the patient.” Be LOUD about yelling “Clear!”

5. PATIENT REGAINS PULSE. Check breathing. If not breathing, what do you do? Rescue breathing: once every 5-6 seconds 6. PATIENT REGAINS BREATHING. High flow oxygen & shock position. Reassess vitals every 5 minutes. Transport!!

IF THE DISPATCH CALL WAS FOR A 9 YEAR OLD… Use pediatric AED pads. Rescue breathing: one breath every 3-5 seconds Depth: ~ 2 inches (1/3 AP diameter) Use peds mask. IF THE DISPATCH CALL WAS FOR A 10 MONTH OLD… Two-thumb encircling hands technique Ratio: 15:2 Depth: ~ 1.5 inches (1/3 AP diameter) For both: be careful of how much air you pump.

THINGS TO REMEMBER: Check if CPR was done before arrival How long was patient down? Past medical history
