PROJECT DESCRIPTION Aims Objectives PROJECT METHODS Inputs - Process Outputs - Results: Current programmes in place that are related to the project.

PROJECT TIMEFRAMES Work plan Estimated timelines when project will commence and end. A schedule with dates of activities to be undertaken. PROJECT BENEFICIARIES Who are they?

Long-term change on societal level IMPACT AND BENEFITS OF PROJECT

How will you monitor your work? How will any lessons learned from monitoring feed back into future planning? How about other external agencies or people? e.g.. monitoring with the help of local Government. Will evaluation of the project be carried out on completion? How will this be done? PROJECT MONITORING AND EVALUATION

If the project is to become self-sustaining – how will this be done? If not, what are the plans for continuance/future funding? If you are providing training, will there be refresher training? How will you know if the skills learned are being used, or will be used in the future? PROJECT SUSTAINABILITY PLAN
