SHARE GROW BELONG 3 The Village Housing for Adults & Youth In partnership with Creative Center Society the Village offers housing for adults with mental health barriers and youth who are homeless or at risk of being homeless. The Villlage – 8937 School Street, Chilliwack BC

SHARE GROW BELONG 4 Adults The Village provides permanent housing for adults with mental health barriers. Adults reside on the 3 rd & 4 th floor Applications and information can be accessed through Creative Center Society. The Village Support 24 hour staff support and supervision Connected to the Youth Support Program Hands On Life Skills Groups Camera system to client ensure safety SHARE GROW BELONG 8 Partnership with BC Housing & Creative Centre Society

Application Process Youth or Social Worker completes and submits an application form Youth completes an interview and demonstrates a source of income If youth meets the criteria they will be given a move in date or placed on a first come first serve waitlist. I If they are not ready to live independently they will be provided with feedback on what needs to be accomplished prior to re-applying Damage Deposit $ & Rent - $ per month SHARE GROW BELONG 9 Partnership with BC Housing & Creative Centre Society

Next Steps Youth meets with Residence Coordinator Dave Herie & Youth Support Worker and reviews and signs a program agreement Youth arranges to have a healthy & safety and building orientation Youth moves in !!! SHARE GROW BELONG 10
