Plan for Continuous Quality Improvement, FY Templates for Local Health Jurisdictions The following pages contain the templates that Local Health Jurisdictions may complete for their CQI Plan. Sites may contact the CHVP Quality Assurance Teams or for assistance completing these plans. The chart-based format is intended to minimize the work necessary to develop a CQI Plan by reducing the actual writing and narration. The CQI plan demonstrates that the CQI process is site-driven and CHVP supported, and provides an opportunity for the CHVP QA Teams and sites to initiate a strength-based CQI process explicitly based on site needs and interests. Sites that wish to submit an alternate plan format should review the document “Required Components of a CQI Plan,” also available on the CHVP website and from the sources listed above.
