Clinical Health Services Services provided through the Health Fee RN assessments and tests for strep, mono, influenza, iron, glucose, etc. General health promotion practices and strategies Counseling services Fees for Service are billed to insurance Medical services provided by Physician Assistant, Nurse practitioner, or psychiatrist Immunizations, prescriptions, procedures, durable medical goods Transportation to health appointments Metro Bus (free) Cab Vouchers (cost billed to student acct.)
SNC Health Requirements 1. Health History Form, 2. Immunizations 3.Health Insurance Students login with their SNC network username (first 4 letters of last name, first and middle initials) and the password they created for their account (e.g. Mary Ann Jones network login is jonema) 1. Student’s Health History Information regarding past health/mental health conditions, allergies, medications and any ongoing treatment (provide plan of care)

Required immunizations Measles, Mumps and Rubella, MMR (2) Tetanus/diphtheria,/pertussis, Tdap (1) Additional Immunizations Hepatitis B (3) Meningitis (Menveo, Bextero); if initial meningitis vaccine was given before age 16, get a booster Include all other immunizations received, especially related to travel outside of US
Requirements (continued) 3.Health Insurance Students will be automatically enrolled in the high deductible plan of the WPS/WAICU student health insurance if no insurance policy is input online (your insurance information is the waiver out of the SNC plan).
