Stereotactic Mammography Image (SMI) Paul Morgan - Fujifilm

SMI – Profile Abstract “Stereotactic Mammography Image is a Content Profile that is designed to provide faithful and complete storage from Acquisition Modalities to Image Managers/Image Archives and retrieval by Image Displays of DICOM stereotactic breast projection x-ray image data, with sufficient display capability to allow optimal visualization of images for the purpose of consultation or second opinion.”

SMI – Problem Statement The Mammography Image profile requirements in the Modality Images Stored and Retrieve Images transactions are not sufficient -to accommodate the desired workflow with stereotactic breast projection x-ray images -on occasions when they are reviewed on non- modality workstations some time after the imaging procedure is completed.

Use Case – Breast Biopsy A patient is scheduled for a Stereotactic Breast Biopsy procedure for the extraction of a suspect mammographic abnormality (e.g., microcalcifications). During the procedure, the technologist acquires stereotactic paired digital mammography projection x-ray images on the Acquisition Modality at prescribed plus and minus angles to a scout image.

Use Case – Breast Biopsy (cont.) The radiologist utilizes these images on the Acquisition Modality to determine the location coordinates for introduction of a biopsy instrument and extraction of tissue from the site of abnormality. Additional stereo paired images are obtained to confirm needle positioning, subsequent placement of a tissue marker, and documentation of successful extraction of the targeted tissue. These single images and image pairs are commonly referred to as “Scout”, “Pre-fire Plus/Minus”, “Post-fire Plus/Minus”, “Post-biopsy Plus/Minus”, etc. After the procedure is completed the images are archived.

Use Case – Breast Biopsy (cont.) If there is subsequent question as to the accuracy of removal of the targeted abnormality, a radiologist needs to be able to view these images on a non-modality workstation (Image Display) for problem-solving and/or second opinion. This requires that the Acquisition Modality record the information identifying the stereotactic images (“Scout”, “Pre-fire Plus / Minus”, etc.) And the correct patient orientation in the images. In addition, the modality will record the targeting information and include image enhancements applied at the modality.

SMI – Value Proposition Provides post-procedural review of Stereotactic Mammography images on general-purpose (PACS) workstations. Leverages many of the requirements for general FFDM acquisition and display specified in the existing Mammography Image profile. Uses existing DICOM definitions to exchange the necessary parameters.

SMI – Relationship to MAMMO Profile Image Display: Re-uses the following requirements enumerated in the Retrieve Images transaction for Mammography images: Background Air Suppression Image Contrast Adjustment Annotation of Image Information Annotation of Size Information Image Size (with respect to the use of Imager Pixel Spacing (0008,1164) and Estimated Radiographic Magnification Factor (0008,1114)) View Actual Pixels. NOTE: Image Displays are not required to support section Same Size or True Size

SMI – Key Requirements Display grouping/Hanging Protocols Images are hung as stereo pairs rather than as a screening set or a laterality grouping The “key” for each image in the hanging order is not its View Code and View Modifier Code, but Value 3 of Image Type (0008,0008), Acquisition Modality required to populate Value 3 correctly Image Display required to use the value as the basis for the Hanging Protocol.

SMI – Key Requirements (cont.) Display Orientation Images are not displayed “chest wall-to-chest wall” as in screening or diagnostic mammography, but with each image of a pair displayed in the same orientation. Acquisition Modality required to correctly populate Patient Orientation (0020,0020) Image Display required to allow the side-by-side display of user selected pairs of images, and accommodate preferences to display the more positive (e.g., “plus” or scout) image on the left and the more negative image (e.g., scout or “minus”) on the right, or vice versa. display a pair of images in the same orientation. allow the user to select or configure hanging protocols such that the chest wall is displayed in particular ways

SMI – Key Requirements (cont.) Annotation of Target Information Target tissue coordinates may be encoded in the Biopsy Target Sequence (0018,2041) for subsequent annotation on displayed images. Acquisition Modality required to encode the coordinates correctly if targeting is used in acquisition. If present, Image Display required to annotate the cursor position and associated labeling (labels, coordinates) on the associated image(s)

SMI – Conclusion Provides for the interchange and clinically relevant display of Stereotactic Mammography images for the purpose of consultation or second opinion.
