Coal Mine Fatal Accident OVERVIEW On June 29, 2011, a 49-year old low structure attendant with 7 years mining experience was fatally injured while watching the electrical trailing cable of a mobile bridge unit. The victim was watching for any problems that may have developed as the system was being retracted (backed up). He was struck in the back by a section of dislodged rib rock, as he fell to the mine floor his head struck the metal frame of the bridge dolly, resulting in fatal injuries.

CONDITIONS OR CIRCUMSTANCES THAT MAY HAVE CONTRIBUTED TO THIS ACCIDENT. OVERVIEW The increased mining height along with the rock parting between the two (2) coal seams created unstable rib conditions. The work position of the low belt attendant (along the narrow side of the belt) placed him in close proximity to the unstable coal pillar (rib).

BEST PRACTICES 1.When excess mining height is encountered or created, rib bolts or some other means of supporting unstable ribs shall be provided. 2.The mobile bridge carriers shall be configured in a manner that positions the electrical trailing cable along the wide side of the belt. 3.Thorough examinations shall be conducted and unsafe conditions shall be corrected prior to work being conducted in the work area.
