LAUSD Background 2nd largest school district in the nation The District covers 710 square miles LAUSD is completing a $19.5 Billion (OCIP II); $1.5 Billion (OCIP III as of 5/1/2013) voter-approved transformation to build new schools, Modernization & Repair phase. The District has 1278 schools The District has built 129 new schools since 1997 LAUSD has completed more than 25,000 modernization and repair projects at existing campuses

LAUSD OCIP Insurance Coverage’s (site specific) $500K SIR LAUSD’s Risk Management department has arranged for the Districts construction projects to be insured under an OCIP; a single insurance program that insures the District, the Board, all enrolled contractors and enrolled subcontractors, and other designated parties for work performed at the Enrolled Project Site(s). LAUSD’s OCIP is Bond funded through Measures approved: BB, R, K, & Y Measure Q upcoming For both New & Existing projects

LAUSD OCIP Insurance Coverage’s (site specific) $500K SIR (cont) Workers’ Compensation (Statutory Coverage) for work related industrial injuries/accidents General Liability ($2 Million) bodily injuries / property damage Excess Liability ($100 Million) Contractor’s Pollution Liability ($50 Million) contamination / spills Builder’s Risk (deductible – policy governs) property damaged during the course of construction

LAUSD OCIP Enrollment Process Alliant is the LAUSD OCIP Broker and handles enrollments Each enrolled contractor shall provide details about themselves and their subcontractors to the OCIP Administrator in order to properly enroll them into LAUSD’s OCIP. The contractor and subcontractor must each complete and submit the Alliant “Enrollment Application” form for each and every contract, even if they already have another contract enrolled on the same project.

LAUSD OCIP Enrollment Process (cont.) Upon receipt & completion of all required data, Alliant will issue to the contractor and subcontractor, a certificate of insurance (COI) evidencing coverage in the OCIP. The completed COI and workers’ compensation insurance policy will be provided to each enrolled Contractor and Subcontractor. Alliant will provide a welcome letter confirming OCIP enrollment specific to the contract number awarded, referencing the workers’ compensation policy issued. Insurance information required by Alliant: 1) workers’ compensation, general liability and automobile for any outside non – LAUSD work, listing LAUSD as additional insured. Master Job Order Contracts – Alliant will need Prime contractor award letter, including contract number, School name(s) and certificate of insurance(s).

Contractor and Subcontractor Required Coverage Contractors and all Subcontractors are required to maintain coverage to protect against losses that occur away from the Project Site or that are otherwise not covered under the OCIP The OCIP places Contractors and Subcontractors into one of two main categories: Enrolled Parties and Excluded Parties Enrolled Parties must provide evidence of Workers’ Compensation, General Liability, and Excess/Umbrella Liability insurance for off-site activities and Automobile Liability insurance for both on-site and off-site activities as specified in the Contract. See Section 2 for the definition of Enrolled Parties. Prime Contractors must provide their Certificate of Insurance to Alliant upon enrollment in the OCIP. Subcontractors must provide their Certificates of Insurance to their Prime Contractor. Excluded Parties must provide evidence of Workers’ Compensation, General Liability, and Excess/Umbrella Liability, Automobile Liability and Pollution Liability insurance for all activities including both on-site and off- site activities as per the insurance specifications in the Contract. See Section 2 for the definition of Excluded Parties. Prime Contractors and Subcontractors must provide their Certificates of Insurance to LAUSD Attn: Larry Chatman.

OCIP Claims Reporting Protocol Complete the report / claim is called into ESIS, with: (instructions in distributed claims kits) 1)Project/School site name 2)Project/School address 3)Employer name 4)Prime contractor’s LAUSD contract number 5)Employer’s OCIP insurance policy number

Claim Reporting Procedures (1of2) Risk Management OCIP claims oversight Receive notice of claim from third party administrator (TPA), job site or blind application Confirm Enrollment in the OCIP Program Confirm scope of work / jobsite listed on policy Contact the owner authorized representative (OAR) / general contractor (GC) to confirm the status of the injured worker Confirm the DWC1(claim form) and MPN Form B is signed and sent to the claim file

Claim Reporting Procedures (2of2) Injured worker is directed to an MPN provider Complete a telephone investigation to eliminate red flags (Monday morning or last day before a layoff) Secure physical evidence, if necessary Maintain a working relationship with all parties approving issues as necessary
