Health Care Proxy (HCP) Applies to Health Care Decisions only if you are unable to make your own decisions. HIPAA Release of Information Instructions to Agent: DNR or Keep Alive at all costs Organ Donation Burial or Cremation Instructions Selection of Proxy Agent and at least one Successor Agent

Durable Power of Attorney (POA) Applies to Financial Matters. Valid if you later become incapacitated Major Gifts Rider (> $500) Choose Your Agent and Successor Agent wisely Agent(s) must accept Valid as soon as executed
