Planning a Training- SEE PAGE 4 OF TRAINING GUIDE Who conducts the training? Time frame for training: 30-45 minute modules, designed for time-compressed provider offices OR three modules can be presented in a 2-3 hour workshop Training settings: primary care settings for provider office staff or larger regional trainings A variety of health care professionals can provide the training. It is best if the trainer has some background on this subject. Consider co-training with another program such as a collaboration between CHDP and health plan staff.

To promote the use of this training, today’s workshop will demonstrate the three modules: Body Mass Index (BMI) Screening Preventive Counseling Clinical Follow-up and Resources
