Today we will…. Discuss employment forms required View sample forms required Discuss the employee and employers responsibilities Discuss what is and what is not valid forms of I. D. 2

Lets see what you know 1. Employers will provide me with all of the necessary forms I need. 2. A high school ID is a valid form of employment eligibility identification. 3. A U.S. Passport is the best form of Id I can provide to prove I am eligible to work. 4. At age 16 I can work in a factory. 5. I need to go to the County Clerks office to get my working papers. 6. My parents need to give permission for me to get my working papers. 3

Employment Certificates AT-18 (Blue Paper) 14 & 15 yr old students for general employment Not Factory or dangerous work. AT-19 (Green Paper) 16 & 17 yr old students for general work purposes Valid for work in a factory or any other trade, business, or service, but not valid for hazardous employment 4

How Do I get My Certificate? Where do I go? Local school district administration What do I need to bring? Written permission from a parent to work. Proof of age, either a birth certificate or some other document at least two years old that satisfies the officer issuing the certificate; A certificate of physical fitness to assure that the young person is in sound health and that the work will not impair his/her physical condition (provided by a doctor) 5

I 9 Continued 7 What do I need? Either: one form of ID that establishes residence and eligibility Example: U.S. Current Passport Or: Two current forms of ID that establish eligibility and residence.

W4 8 This form is needed so that the employer knows the correct amount of federal income taxes to withhold
