Diabetes Things that are currently working Teenage clinics 15 to 18 in an evening. Transition plans commenced with young person from 13years old. (use goals of diabetes education resource pack and transition plan resource from North West diabetes group). Looking to do yearly as part of annual review. At the teenage clinic the young person is encouraged to come into see team on their own without parents. Parent invited in at end of consultation if requested. Gradual process at the young persons pace. Transition clinic twice a year at Chorley and Preston. Adult and paediatric team at clinic. Now done in familiar environment. Young person given the choice at having the PDSN attend at the 1 st appointment in young adult clinic.

DATA COLLECTION Collecting 3 monthly Hb1AC results (long term test that shows if diabetes management is on target). Collecting DNA info re clinics monthly. Looking at DKA (diabetes ketoacidosis) admissions yearly and individually. (frequent attenders) would be good to look at DNA info for after transition to young adult clinic!

CHALLENGES Finding time to do transition plans. Adult team have limited time /personnel Unable to attend paediatric clinic to meet young person before transition date. Looking to have a joint diabetes structured education session where the young person would meet the adult team.

Using sanofi ice breaking cards to aid discussion in clinic for the 13 to 15 year olds. This has been helping to make the clinic appointment about their agenda. It has also promoted the young person to speak in clinic as parents tend to do all the speaking. We have had a few surprises of what the young person picks and has opened up some difficult conversations.

Asking young people from 15 years on to write down 3 questions for diabetes team on 3 question post its. Helps initiate their agenda and sets the scene for the clinic visit. 4 of our young people attended the ambassador course awaiting feed back. Young people reported they had enjoyed the weekend.

NEXT STEPS To look to progress to having a patient event day with the epilepsy team, and complex needs team using the expertise of the patient support team. Video interviews of patients. Continue using ready steady go paper work to help transition process. To have group education session with 16 to 18 year olds looking at independent living and teenage issues jointly run by adult and paediatric team
