NM MESA INCENTIVE AWARD

What is the NM MESA Incentive Award? o Award for eligible graduating seniors that have been in NM MESA for their 11 th and 12 grade years (other criteria also need to be met) o Cash award of up to $1000 o Award is taxable; it is not a scholarship  Students earning $600 or more will receive an IRS 1099 Form in January

Students Earn Points as NM MESA Members o Middle School  Points for being in NM MESA o High School  Points for NM MESA activities and academic achievement o Points earned = Money earned 3

Timeline for Incentive Award o May 1, 2014  NM MESA requests official senior transcripts from all school registrars.  Seniors should request official college transcripts sent to NM MESA for any college and/or dual credit courses o July 25, 2014  Seniors eligible for an Incentive Award notified by and USPS of their calculated amount  Online application available on NM MESA website:  Seniors need the following information before filling out the application: NM ID number (State PED #--obtain from school registrar or NM MESA Advisor) Fall schedule of college classes (electronic copy to upload) Federal Tax Form W-9—student information and signature (not parent information--scanned copy to upload) Student testimonial (electronic copy to upload from Word) 6

Timeline for Incentive Award o August 6, 2014  Deadline for appeals to NM MESA Regional Coordinator at 5 pm MST o August 31, 2014  Deadline for submittal of online application at 5 pm MST o September 5, 2014  Final Incentive Award determinations made o September 8, 2014—week of  Checks cut and mailed to students 7

Appeals Process o Contact NM MESA Advisor or Regional Coordinator if you disagree with calculated dollar amount sent to you o If Advisor or Regional Coordinator agrees, request a signed letter explaining the discrepancy with supporting documentation attached o Provide this letter and documentation to your NM MESA Regional Coordinator no later than August 6, 2014, 5 pm MST o When your appeal has been reviewed, you will receive a written determination of the outcome. All determinations are final. 8
