Health Insurance TYPES OF COVERAGE PROVIDERS DESCRIPTION Health Maintenance Organization (HMO) HIP, Aetna HMO, CIGNA, Empire Personal Care Physician (PCP) Need referrals Little or no out of pocket expense Exclusive Provider Organization (EPO) Empire Must stay in network only No referrals No Primary Care Physician (PCP) Higher Bi-weekly premiums Point of Service (POS) HIP Prime POS In/out of network flexibility Deductibles Preferred Provider Organization (PPO) GHI- CBP/EBCBS Freedom to choose Deductibles for out of network Negotiated lower payments Nila CUNY offers several health insurance options to its employees and today we will attempt to define a number of acronyms as they related to our health insurance. Lets look at: HMO. Health Maintenance Organization is a system of health care that provides managed, prepaid hospital and medical services to its members. If you are a member of an HMO, you will need a referral to go to a specialist. EPO. Exclusive Provider Org. offers a higher level of choice and flexibility than many other managed care plans. Members can see any provider in the EPO Network. There is no out of network coverage. POS. Point of Service allows employees to see a doctor in and out of network. Going out of network will require a deductible. PPO. Preferred Provider Org is a plan that offers the freedom to use a network or an out of network provider for medical and hospital care. Using a non-participating provider is subject to deductibles and/or coinsurance. Due to negotiated lower payments, this PPO is a less costlier option than POS.
Health Insurance (cont’d) The Summary Program Description Manual describes City of New York health benefits program in detail. www.nyc.gov/olr Open Enrollment Period this year is scheduled for: - Fall 2012 (TBD) Nila For more information about your benefits, you may want to review The Summary Program Description Manual and the PSC/CUNY Welfare Fund which describe CUNY’s health benefits program in detail. You need to speak to your current health care providers to see what they take, before signing up and of course, the cost (bi-weekly) Also, the Open Enrollment Period is scheduled for : Normally in November, TBA This is a time when employees can add or switch plans.
Pension – Irrevocable Choice NYC Teacher’s Retirement System (Tier VI) Optional Retirement Program TIAA –CREF (Tier VI) Defined Benefit Plan: Benefits based on age, average of highest five years consecutive salary and years of employment Defined Contribution Plan: Benefits are based on the amounts contributed by the employer and employee and the success of the employee’s choice of investments Vesting: 10 years of full-time credited service Vesting: After 366 days of continuous full-time employment (immediately if the employee has prior vested TIAA-CREF retirement contract.) Contribution: 3% until 4/1/13; then contribution rate depends on salary. Contribution: Employee Contribution 3% until 4/1/13; then contribution rate depends on salary, Employer contributes 8% for the first 7 years and 10% thereafter. Retirement Age: 63 with unreduced retiree health benefits. No retirement age, however needs 15 years of continuous F/T service and be age 62 for unreduced retiree health benefits. Nila Prior Board of Ed, NYCERS, or TRS members are suggested to choose TRS in order to maintain your membership service and city start date.

Nila Voluntary savings program – several options –online enrollment (provide proof for 403b enrollment) Supplemental Retirement Annuity (SRA) is a 403(b) benefit that is administered by TIAA-CREF. This benefit is available to HEO Series, Classified Managerial and Faculty: This year those employees who choose to use this benefit can contribute a maximum of $16,500 pre-taxed dollars. 457 (b) is a Deferred Compensation Plan that is sponsored voluntary retirement savings plan offered to all public employees. It is overseen by the New York State Deferred Compensation Board (the Board) and managed by professional staff. There is no conflict of interest in enrolling in 457b and 403B so you can have both TDA’s at the same, however you cannot have a 401K with a 403B The benefit of a Tax Annuity plan is to reduce your income tax liability.

LEAVES Family Medical Leave (FML) Paid Parental Leave (PPL) TYPE OF LEAVES Family Medical Leave (FML) Paid Parental Leave (PPL) Scheduled Holidays Unscheduled Holidays Annual Leave Temporary Disability (sick) Jury Duty Andrea CUNY also provides a variety of leaves such as: Paid Parental Leave is an eight (8) week-leave for HEOs and Instructional staff with at least one (1) year service and is paid only after the arrival of a newborn. (until Dec 31, 2011) FMLA available to all employees who have worked a minimum of 1250 hours (1 year of service) Four days of Unscheduled holidays this year. (only HEOs and CLTs) Temporary Disability Leave which s the same as sick leave – Leaves beyond 5 days is considered FML leave if you meet eligility, need medical documentation in any case Leave to serve on Jury Duty. Full-time employees who serve as jurors receive their regular salary provided they furnish the HR office with a copy of the summons to serve. and the certificate you received from the Court saying the duration and date of the time served. CUNY gives twelve (12) scheduled holidays per year, and is the only organization that gives it employees, “Lincoln’s Birthday” off. Annual leave period - End of Spring Session 1 – check academic calender begins when “Web grades due” and ends when “faculty returns”. Library – 20 days for first year, additional day for each year up to 30 days Dedicated Sick leave enables individuals who are employed fulltime on an annual salary basis to donate sick leave and/or annual leave for use as sick leave by a seriously ill or injured eligible employee who has been designated by the donor

This program provides a flexible, convenient and low-cost way to save for Higher education expenses. You can save for a child, grandchild, or friend Tuition Fee may be waived for undergraduate and graduate courses at any college of the City University of New York on a space available no cost basis. CUNY Work/Life program is administered by Corporate Counseling Associates (CCA). It provides counseling and consulting services in life event stress, emotional well-being, workplace challenges, legal and financial difficulties, alcohol and substance abuse, and dependent care concerns. This benefit is available to you and your family members. A 24-hour helpline is available. You are covered for Workers Compensation benefits if you suffer a job-related injury or illness. You must seek emergency medical treatment immediately and contact the HRD Office to complete forms and provide medical documentation pertaining to the injury.
