Nursing Process- Evaluation

Evaluation Evaluation measures the client’s response to nursing actions and progress toward achieving health care goals. The emphasis is on client outcomes.

Dynamics of Evaluating Nursing Process Compares the observed results of the interventions Uses critical thinking to analyze the results Evaluation process continues until problems are resolved

Evaluation of Goal Achievement Why evaluate? Evaluate initial assessment (baseline) to determine whether desired changes have occurred Determines if the goals purpose was accomplished Varying degrees of goal achievement may occur

Evaluative Measures and Sources Simply the assessment skills and techniques used to collect data Primary source is the client

Expected Outcomes and Goals Expected outcomes are the step by step responses or behaviors of the client to remove or modify the nursing diagnosis Goals are the summary statement of what is to be accomplished

Care Plan Revision Unmet and partially met goals require revision of the nursing care plan Discontinuing a care plan If a client and nurse agree that the expected outcomes and goals have been met then the care plan is discontinued

Care Plan Revision Modifying the care plan Reassess Nurse then reevaluates all nursing diagnoses and determines whether the diagnostic statements were accurate Re-evaluate the goals and expected outcomes Evaluate the nursing interventions

Evaluation of Health Care Facilities Hospitals and health facilities have and need ways to evaluate the effectiveness of their organizations and the care given within the organization JACHO Defines QA (quality assurance) as planned and systematic process for the monitoring and evaluating the quality and appropriateness of client care and for resolving identified problems QI= Quality Improvement QA and QI are used to evaluate clinical practice and management of their facilities
