Page 3  Three Medicare supplement plan insurance policies are typically available from Aetna Life Insurance Company (Aetna): A, B and F. Exceptions:  Plans A and F are available in Nevada  Plans A, B, C and F are available in Michigan and Texas  Can be purchased in addition to Medicare Parts A and B to receive increased benefits, such as coverage for your Part A and B deductibles and coinsurance payments  Allows you to visit any licensed doctor or hospital that accepts Medicare  Provides protection from many of Medicare’s major out-of-pocket expenses  Guaranteed acceptance, regardless of your medical history, when you apply during the first six months you are enrolled in Medicare*  No waiting periods if you have pre-existing conditions  Your policy cannot be cancelled as long as you continue to pay your monthly plan premiums Aetna Individual Medicare Supplement Plan SM Insurance Policies (Medigap) * Other guaranteed acceptance enrollment periods may apply.

Plus, you can enroll in an Aetna Medicare supplement plan insurance policy any time of the year! If you live in a state where our Medicare supplement plans are available, and you know that you are eligible for the plan, you can request an enrollment packet by calling , ext Then follow the steps below. Complete the enrollment form. Step 1Step 2 Mail your completed form, as well as a check for your initial premium, to Aetna. Step 3 Review your enrollment kit and select a plan.
